Long-term Conditions

What are long-term conditions?

Long-term conditions or chronic diseases are conditions for which there is currently no cure, but which can be managed with drugs and other treatments. They include a broad range of medical issues e.g. asthma, diabetes and osteoarthritis.

In addition to disease specific symptoms, they can cause invisible symptoms such as pain, fatigue, and mood disorders. Good management of your long term condition should allow you to remain relatively stable and enjoy a better quality of life.

In July 2020 we are starting to roll out a new programme for annual review of some long term conditions which require regular monitoring, called Personal Health Planning.
