New Patient Registration

register_nowAlthough patients are registered with the practice as a whole, if a preference is requested for a particular individual, we will record that preference and endeavour to comply, provided such individuals perform in the desired service.

If you live in the practice area and wish to register for NHS general medical services please drop into the Aviemore Health Centre and speak to the receptionist. You will need to bring two forms of ID (including 1 piece of photographic ID) to complete your registration.

You may print of the application form and bring it along to the practice when you first visit.

Registration Form

When you register with us we think this is a good opportunity to discuss your health and get to know your medical problems and requirements if there are any. Health maintenance is important for all of us and to this end, newly registered patients are required to see one of our team for a Health Check.

Please print off the New Patient Health Check form and bring it along to the practice with your Registration Application form.
